Average quality can hide unacceptable risk
A scorecard is useful because it creates consistency. It becomes dangerous when the total score is treated as the only decision rule.
Imagine a training asset scores 94 out of 100. Most sections are excellent, but one assessment answer is objectively wrong. Or a key claim cites a source that does not exist. The numerical average still looks strong, but the material should not be released.
Some defects are gates, not deductions
Quality systems work better when they distinguish between scored controls and critical controls.
- Scored controls help compare overall quality and identify weaker areas.
- Critical controls define conditions that must be satisfied before release.
Examples of critical failures can include fabricated sources, materially incorrect facts, invalid assessment keys, privacy or confidentiality breaches, missing required approvals, or a release with no named human owner.
Use an override rule
The decision logic should be explicit: if a critical control fails, the result is Hold or Revise regardless of the total score.
This removes a common failure mode in QA systems: compensating for one serious defect with many minor strengths. Ten well-formatted sections do not make a false safety instruction acceptable.
Keep the score — but give it the right job
The total score still has value. It can help teams monitor quality trends, compare versions, identify recurring weaknesses and prioritize improvement. It should simply not be allowed to neutralize a release-blocking issue.
A practical hierarchy is:
- Check critical controls.
- If a critical control fails, stop release and correct it.
- If critical controls pass, use the total score to determine whether the asset is ready or needs further refinement.
Good governance is visible in the decision rule
When everyone knows which issues are non-negotiable, review becomes faster and less subjective. The goal is not to make QA bureaucratic. It is to make the release decision defensible.
